What I Got Wrong About Reducing No-Shows Before We Fixed It
I thought no-shows were a reminder problem. They're not, and the fix that actually worked looks nothing like sending more texts.

The Problem
Every practice I've talked to has a no-show number they don't love, and almost none of them can tell me exactly when those no-shows happen without pulling a report first. "How do I reduce patient no-shows" is one of the first questions I hear on almost every discovery call, and that gap, between knowing you have a problem and knowing precisely where it comes from, is where most of the money actually leaks out.
What I Thought
My first assumption was the obvious one: send more reminders. A text the day before, maybe a call if the appointment was big enough. That felt like the whole fix, because it's the fix almost every practice management system already ships with.
What Actually Happened
Reminders alone moved the number a little, then stopped. The patients who were always going to show up got reminded twice. The patients who were going to miss the appointment anyway still missed it, just with slightly better notice. The reminder was answering the wrong question. It assumed the problem was forgetting, when for a real share of no-shows the actual problem was that the patient meant to reschedule and never found an easy way to do it.
What I Learned
A no-show isn't one problem, it's usually two: forgetting, and friction. Fixing the first without fixing the second gets you a flatter number, not a fixed one. The practices that actually moved their no-show rate were the ones that made rescheduling as easy as the original booking, and that followed up on every no-show within a day instead of waiting for the next scheduled reminder cycle.
What I'm Building Now
